You can use axf:suppress-duplicate-footnote in XSL Formatter V3.3. Deters the display of duplicaited footnotes on the same footnote region when the same footnote is assigned to two or more places in the same page. Please see the Online Manual [Extensions] - [Footnote] for detail.
